Teacher Recommendation Form

Parents and students interested in joining 2/3 Dance Club need to print and deliver this form to your teacher from last year. Your previous teacher will fill it out and give it to Mrs. Patterson or Mrs. McWay. If you need a copy of the Teacher Recommendation Form, please feel free to stop by Mrs. Patterson's classroom in K-1 to pick one up. Failure to give this form to your previous teacher may result in you not being able to participate in Dance Club.
